Walk into any supplement aisle and creatine sits there in a tub the size of a paint can, marketed almost entirely to men chasing bigger lifts. That's odd, because creatine monohydrate is arguably the best-studied performance supplement in existence, with decades of trials behind it — and a growing slice of that research is specifically about women. The findings aren't a simple copy-paste of the male data, and treating them that way is where most of the confusion around "does creatine work for women" actually comes from.

What creatine is doing inside the muscle

Creatine works through the phosphocreatine energy system. Your muscles store a small reserve of phosphocreatine, which donates a phosphate group to rebuild ATP during short, intense efforts — a heavy set of squats, a sprint up stairs, a max-effort deadlift. Supplementing with creatine raises that intramuscular reserve, which means you can push out another rep or two before the system runs dry. More reps at a given load, over months, is what eventually turns into more strength and more muscle. That's the entire mechanism, and it's one of the few supplement claims in sports nutrition that's genuinely settled science rather than marketing.

The dose that's been tested, not the one that sells more tubs

Skip the loading-phase protocols still printed on a lot of packaging. Research supports a flat 3–5g per day of creatine monohydrate, taken consistently, with no need to front-load with 20g/day for a week first — that older loading approach saturates muscle stores a few days faster, but the endpoint after a month is the same. Take it with food if plain water gives you a stomach ache; splitting the dose across two meals fixes GI discomfort for most people who get it. Buy creatine monohydrate specifically. The buffered, micronized, and "HCL" variants sold at a premium haven't shown any advantage over the plain powder in head-to-head trials, and paying more for them is a waste of money.

Where the data actually splits by sex

This is the part that gets glossed over. A systematic review and meta-analysis covering 35 randomized trials and 1,192 participants found that men gained 1.46kg of lean body mass on creatine compared with placebo (95% CI 0.47–2.46), while women gained a non-significant 0.29kg (95% CI −0.43 to 1.01). A separate 2025 meta-analysis looking specifically at squat strength found a similar pattern: men improved by a weighted mean difference of 6.43kg, women by 1.63kg. Researchers point to testosterone-linked differences in baseline muscle mass and possibly in creatine uptake itself as the likely explanation, though that's a working hypothesis rather than a closed case. What these numbers don't capture is trial length, and that turns out to matter enormously — most of the studies feeding these meta-analyses ran eight to twelve weeks, which is barely enough time to establish a habit, let alone drive meaningful hypertrophy in anyone. Pool a bunch of short trials together and you'll systematically undercount the response in any group that needs longer to show it. Women, as it turns out, are exactly that group.

None of that means creatine doesn't work for women. It means the average effect size in mixed-duration trials is smaller and noisier for women than for men — a very different statement, and one worth sitting with before you write off the supplement.

A plausible reason why: estrogen and the creatine transporter

One mechanism getting attention is that estrogen appears to downregulate expression of the creatine transporter — the protein that shuttles creatine from the bloodstream into muscle cells. If that holds up, women, particularly during higher-estrogen phases of the cycle, may not saturate their intramuscular creatine stores as efficiently as men do on the same dose. It's an early explanation, built mostly on smaller mechanistic studies rather than large trials, but it fits the pattern seen in the outcome data and gives researchers a specific target for the next round of dosing studies.

Duration matters more for women than the label suggests

Here's where the picture improves. A meta-analysis focused specifically on older women found that creatine significantly increased upper-body strength (7 studies, n=142, p=0.04), with the effect concentrated in trials lasting 24 weeks or longer — shorter interventions showed essentially nothing. Lower-body strength and total muscle mass didn't reach significance in that same analysis. Take that as the real-world guidance it is: an eight-week trial run of creatine tells you almost nothing about whether it works for you. Give it six months alongside a real resistance training program before deciding it isn't for you.

The water-weight myth doesn't hold up under a scanner

It's not water weight.

The most common objection to creatine among women is that any weight gain is "just water." Burke and Candow's 2023 analysis using MRI and CT imaging — which sidesteps the water-retention confound entirely by measuring actual tissue — put the probability of genuine muscle hypertrophy at 96.1%. There is a water component early on, but it's not the bloated, under-the-skin water people picture. Creatine pulls fluid into the muscle cell itself, which is what triggers the cell-swelling signal that activates satellite cells and drives real hypertrophy over time. That's a fundamentally different process from subcutaneous water retention, and conflating the two is where most of the "creatine makes you puffy" fear comes from. Pairing creatine with carbohydrate does increase muscle glycogen storage and, with it, a bit more of that same intracellular water — which is a sign the muscle is stocking fuel, not a red flag. If your jeans fit the same a month in and the scale is up half a kilo, that's very likely what's happening, not fat gain and not bloating.

What the menopause research shows so far

A small pilot study followed fifteen women — five in perimenopause, ten postmenopausal, average age 54 — through 14 weeks of twice-weekly total-body strength training paired with creatine supplementation. Researchers tracked body composition with a Bod Pod, estrogen via an estradiol spit test, cognitive performance, weekly mood and sleep questionnaires, and strength on an isokinetic dynamometer. It's a small sample, and nobody should treat it as definitive. But it's one of the only studies designed around this specific life stage rather than lumping perimenopausal women in with twenty-year-old athletes, and the fact that it exists at all signals where the research is finally heading.

Mood, sleep, and a genuine contradiction in the data

A 24-week randomized, double-blind trial split older women into four groups — placebo, creatine, placebo plus strength training, and creatine plus strength training — and tracked cognitive and emotional measures throughout. By week 24, both trained groups showed significant improvements on the Geriatric Depression Scale compared with the untrained placebo group. Notice what that result actually says: the mood benefit tracked with training, not with creatine specifically, since the untrained creatine group didn't outperform untrained placebo. That's an important distinction, and one most headlines skip entirely when they report "creatine improves mood in women" as if the supplement did the work on its own. Separately, a study on acute sleep deprivation found that a single 0.35g/kg dose of creatine improved cognitive performance measured at 3, 5.5, and 7.5 hours later, suggesting a short-term brain-energetics effect distinct from the long-term muscle story. Two different mechanisms, two different timelines — don't assume the mood benefit and the cognitive benefit are the same phenomenon. One is downstream of the training itself; the other looks like a genuine, acute effect of the compound.

Is it safe, and is it worth taking

Creatine monohydrate has one of the longest safety track records of any sports supplement, and kidney function markers stay unaffected in healthy individuals across every major long-term trial to date. The people who should actually check with a doctor first are those with pre-existing kidney disease — for everyone else, the safety conversation around creatine is largely settled.

Take it daily if you lift weights two or more times a week. Skip it if you're only doing light cardio and never train close to failure, since the mechanism it works through simply won't get called on. Stick with plain creatine monohydrate, 3–5g a day, and commit to at least six months before you judge whether it's doing anything — the trials that found real strength gains in women were the ones that ran long, not the ones with the flashiest loading protocol.
