You've already fixed the obvious things. Bedtime is consistent, the phone stays out of the bedroom, and you're closing in on eight hours most nights. And yet the fatigue that used to lift after a deload week is still there — the bar feels heavier, the sets are the same, and your legs feel three days into a cold that never quite arrives. If you've quietly assumed you just need more sleep or a lighter block, there's a blood marker worth checking before you touch your programme again: ferritin.
When "Just Overtrained" Doesn't Explain It
Overtraining and low iron produce an almost identical list of complaints, which is exactly why so many active women spend months chasing the wrong fix. Both show up as flat lifts, a resting heart rate that's crept up five or six beats from where it usually sits, and recovery that takes an extra day no matter how well you eat or sleep. But a few signs point specifically toward iron rather than programming fatigue: breathlessness climbing a flight of stairs that never used to bother you, restless legs at night, nails that chip and split more than usual, and — this one surprises people every time — a craving to chew ice. None of these show up on a standard "how's training going" check-in, and none of them respond to an extra rest day. Coaches often tell clients to back off volume for two weeks and reassess; if fatigue this specific hasn't budged after that, the next step isn't a lighter block. It's a blood test.
Why Active Women Are the Group Most Likely to Run Low
Three things stack on top of each other for women who train hard, and any one of them alone would be manageable. Menstrual blood loss is the baseline: the recommended dietary allowance for iron sits at 18 mg a day for women aged 19–50, more than double the 8 mg set for men, precisely because monthly blood loss has to be replaced from diet. Add repetitive-impact training — running especially, but any high-volume lower-body work counts — and you get footstrike hemolysis, where red blood cells rupture from repeated ground contact and get cleared out faster than the body can replace them. Then there's the mechanism almost nobody mentions outside sports-nutrition circles: a hard session triggers a spike in a hormone called hepcidin that blocks dietary iron absorption for roughly three to six hours afterward. Time a post-workout meal around that window and you can eat a plate of spinach and lentils and absorb almost none of the iron in it. Stack a heavy training week, a period, and a habit of training fasted or eating light after sessions, and it's not surprising that reviews of endurance athletes put iron deficiency somewhere between 15% and 35% of women, depending on the sport and the training load studied.
Deficiency Isn't the Same as Anemia — and Most Panels Miss It
Normal hemoglobin does not mean normal iron.
The World Health Organization defines anemia in non-pregnant women as hemoglobin below 12 g/dL, and that's the number most standard blood panels flag as a problem. Ferritin, the protein that reflects your body's stored iron, is a different marker entirely — and it can be sitting in single digits while hemoglobin still looks perfectly fine, because the body draws down its reserves for months before red blood cell production actually falls. That gap is exactly where a lot of training fatigue lives. A GP who orders a routine full blood count and says "everything's normal" may genuinely not have looked at ferritin at all, since it isn't part of a standard CBC and has to be requested separately.
Getting Tested: What to Ask For and When
Ask for a ferritin test by name — not a full blood count, not "iron levels checked" as a vague request, but ferritin specifically, alongside serum iron and transferrin saturation if your clinic offers a full panel. This matters most if any of the following applies: periods heavy enough to soak through protection in under two hours, a vegetarian or vegan diet, more than six hours of endurance training a week, or a performance plateau that's held for over a month despite adequate sleep and calories. Many sports-medicine clinicians consider anything under 30 ng/mL suboptimal for an athlete, even though a standard lab reference range might list "normal" as low as 12–15 ng/mL. That gap between "technically normal" and "adequate for someone training six days a week" is where symptomatic but undiagnosed deficiency hides, and it's worth pushing your GP on if the first answer you get is a shrug.
Food First: Heme, Non-Heme, and What Actually Blocks Absorption
Iron from meat, called heme iron, absorbs at roughly 15–35%, while the non-heme iron in plants and fortified foods absorbs at a much lower and more variable rate — often under 10%, depending on what else is on the plate. That doesn't make plant sources pointless; it makes what you eat them with the whole game.
- Heme sources worth building a meal around: beef, lamb, and dark-meat poultry, plus liver if you can stomach it once in a while — nothing else comes close for absorption efficiency.
- Non-heme sources: lentils, chickpeas, tofu, pumpkin seeds, and fortified breakfast cereals.
- Vitamin C is the single most reliable non-heme booster — a glass of orange juice or a handful of raw pepper strips alongside a lentil dish can roughly double how much iron you actually absorb from it.
- Calcium taken at the same meal — a big glass of milk, a calcium supplement — competes with iron for absorption and blunts it.
Tannins are the one most people have never heard of: a cup of tea or coffee drunk alongside an iron-rich meal can cut non-heme absorption by more than half. The fix isn't giving up coffee — it's moving it to an hour before the meal or two hours after, rather than drinking it with your lentil bowl. That rule matters far less if red meat is already a regular part of your diet, since tannins interfere with non-heme absorption specifically and barely touch the heme iron in a steak.
Supplementing Without Testing Is the Real Mistake
Once low ferritin is confirmed on paper, an iron supplement recommended by a clinician is a fast, well-studied fix — most people see ferritin climb meaningfully within eight to twelve weeks on an appropriate dose. Guessing your way there without a test is the part to avoid. Iron supplements taken without a confirmed deficiency commonly cause constipation and nausea for no benefit, and in rarer cases can push someone toward iron overload — a real risk for anyone with hereditary hemochromatosis, a genetic condition that's more common in people of Northern European descent than most assume and that a simple blood test would have flagged before a single capsule was swallowed. Don't start supplementing because a training partner swears by it or because a bottle at the pharmacy says "energy support." Get the ferritin number first, supplement to a target with someone tracking it, and retest in a few months rather than guessing when to stop.
The gym you're doing everything right in won't fix a ferritin count sitting at nine. That fix starts at a blood draw, not another deload week.
